I think a lot of comorbidities are under reported in the statistics and are actually much higher because they require disclosure. And it has been my wife's experience that people don't check all the boxes when they come to the emergency room. During her residency they would ask a patient if they were using drugs, they would say no, then they would run their blood and it would come back positive for marijuana, cocaine or benzodiazepenes (or all three depending on the patient) about 3/4 of the time (this is pretty important for dentists as anesthetics can react with cocaine and stop a person's heart). I figure cigarettes, obesity, alcoholism and drug use are all probably under reported in the stats. Controlled diabetics and high blood pressure are probably well marked as they can test for those or will know based upon drugs they have to administer.
