It's been about 2 years for me. Pretty much 100% although it still gets sore during high mileage weeks. The first year was bad. Anything faster than 6:00 pace would really aggravate it. From the 12 month point to 18 months it got much better.

More than six years for me. I got mine in Dec 2009 and it's still not 100%. I just aggravated it again last month with too much kicking in Tae Kwon Do.

No surgery.
Took two months completely off and only did leg and ab light strength exercises. Then, gradually built back mileage over next two months.
