Huh. My experience is the opposite. In college, all students called our professors Dr. and last name. I have a friend who is a retired professor, and pretty much everyone still calls him Dr. and last name.
I almost always called my profs "Dr" if they had a PhD unless they told us otherwise. I had a former prof do some work for me at workshops and would still introduce him as Dr.... Now after the workshop I would call him by his first name, but I would have never done that as a student.
The JD only came into existence in the 20th century in the US. Funny that Australia considers it a masters degree which seems more appropriate
