Based on his fitness I would venture to guess the HM effort in Prague wasn't actually that hard of an effort and the time was indicative of him having to hold back because of the foot injury. If he did the 22 miler a few days after the HM, maybe a day or two after the cortisone shot, then did the 10 miles of intervals on Thursday or Friday of that same week he then still has about 8 days to taper and freshen up for Boston.
You have to consider the fact he has access to basically every piece of technology and therapist beneficial to recovery. For that reason alone I would expect him to be able to stack this type of workload better than the regular 2:30 guys using their bathtubs to ice in.
