I'd guess that a 100 miler is easier on your heart. Jogging along at ~50-60% hr max for 100 miles is probably easier on your heart than a marathon at 80-85% max.
There is currently a large-scale study underway investigating exactly that question.

Conclusive evidence will not be in for several years, but at the study's outset, the participants (ultramarathon runners) had a lower incidence of most major health problems, including heart disease, than average.

What do 100 milers do? They kill off your brain because you have to have no brains to run them in the first place.

One of the summaries, Cardiac Function, states that the right side of the heart shows the most adaptations. That is contrary to everything I have ever learned and makes little sense. The right side pumps blood to the lungs which are pretty much right next to the heart. The left side (left ventricle specifically) pumps blood to the body and has a higher resistance to push against.
