I had a good experience with the team sales at Eastbay. They give reduced prices for bulk sales (not sure if this applies to orders as small as yours). All you do is email them the image you want to have put on the singlets and they can do the screen printing as well.
If you order 6+ jerseys you can get them from anacondasports.com for pretty cheap. Then shop around and find the most reasonable screen printer around and give them your logo of choice. I recently did this for 6 jerseys and we purchased the most recent Adidas team track singlet. After the cost of the jerseys, shipping and printing were added up the singlets still were less than if we were to buy just the plain jersey retail.
