It's up there with soccer; outside of the Vatican, you'll be hard pressed to find a country that didn't have an athletics federation. There are more national athletics federations than UN members.

Keep in mind that a number of the member federations are not independent nations (Puerto Rico, Macao for example) and others are in dispute (Taiwan).
You can´t really compare the two. Track&field is not ONE sport, but many and many of them have very little in common (shot putting and distance running don´t have anything more in common than distance running and weight lifting).
The football ("soccer") world championship is the second biggest sports event in the world after the Olympic games. It has A LOT more TV spectators than the track&field world championship.

I'm actually not sure about this. The World Cup doesn't involve as many athletes but in some ways I'd bet it is "bigger" than the Olympics. I'd be interested to see stats on viewership, global interest, etc. I wouldn't be surprised if the World Cup could reasonably be termed "bigger" than the Olympics.

The nature/design of the Summer Olympics is much more massive than the World Cup. Overall, the Summer Olympics draws more athletes and viewship. But the World Cup final has the highest viewship for a single competition.

Actually, the most watched sports events in the world are the Olympic Games, Soccer World Cup, and the IAAF World Championships, in that order.
Track and Field is probably "played" in more nations than soccer, and probably "more commonly known" in nations that don't care that much about soccer, such as Russia, China, and the US.
