A big reason is that, for Americans, we encounter a psychological block at 5:00/mile. So when runners start seeing splits in the 4:xxs, they back off.
Every other country doesn't run into a barrier like that until 3:00/km (4:48/mile).
That's the difference between 2:11 and 2:07.

The problem with any comparisons and mental hijinks is that you don't know what chemicals your comparison target is on.

Not if I can help it.
Hall was a legit 2:06 guy which is an awesome time for a clean athlete. There is no definitive proof that all of these runners in the 2:03-2:04 realm are clean. Some may be, however not sure they all are as many have no track credentials to back up the corresponding marathon times they run.
Rump, like him or not should be able to run 2:05-2:06 on a course like Berlin or London.
