When my daughter was born I was up all night in the hospital. In the morning, everyone asleep, I headed home to eat/shower/sleep - stopped to eat, and I had a beer with steak and eggs. Was one of the best beers I ever had. So I had another. Then went home and slept for 9 hours
Maybe the guy was a westbound traveler and had just flown into town. If he was from Europe, it would have been late afternoon or evening from his perspective.

Well I couldn't hold that schedule either which is why I found it interesting. Look back at what I wrote as the OP and you'll see that I was not critical in any way. I was asking for speculation on what situations this would seem normal.
So far the idea of the off-duty surgeon seems like the most noble proposition.
