4) Even when dopers do have the same access to drugs and/or methods, dopers in some countries can dope to a greater extent (more drugs, higher doses, longer duration, etc.) without worrying as much about detection, since anti-doping in those countries is still significantly less effective and less efficient than in many others for various reasons that may include but aren't limited to: A lack of resources/technology for testing, Less importance placed on legitimate detection, Geographical/transportation barriers and other barriers that make it harder for testers to complete legitimate random unannounced tests (with no advance warning), etc, etc.
If there's not much of risk of a truly random unannounced test where the athlete is training, there's not much of a deterrent to full scale doping.
You can't even compare the level of testing that athletes with Salazar have in relation to athletes with Jama Aden.
For Aden's athletes it's a free for all, and that's where Dibaba, Makhloufi, Mo Farah, etc are coming from.
Sure, Salazar is dirty, but he's operating under the strictest anti-doping in the world. And guess what, his dirtiest athlete can't compete with the guy that just goes around the world getting the drugs from the best cheaters in the World. Mo goes to Salazar and gets his stuff, goes to Aden, gets his stuff, goes to Font Romeu and gets the stuff from Paula's doctors.

You want to turn a sport that is great because literally anyone can do it, and turn it into an event where to get to the top you need to have the best drugs and doctors behind you? You want parents to be put in a position where they might think, actually I don't want my child to go into athletics, you've no chance of getting to the top unless you take drugs and I don't like the health risks that come with that?

A few questions. Where are all of these drugs being made? Who is making all these drugs? Who is supplying the athletes with these drugs? Who is facilitating the massive coverup which makes it possible for all of this cheating you speak of to take place?

This is mindset is the problem. The US team never used to be content with silvers & bronzes, never mind top 5 or top 10. It's all about winning and the US team is currently doing a terrible job at that.

Of course the majority of athletes are not doping but they are not qualifying to worlds because of the few that are.
Dibaba might be more talented, but she might just be on something better. Or respond better.
