Looks fine from the waist down. As others have said, from the waist up your form, while not bad, is that of someone who once had a large/powerful upper body. You've lost the upper body due to distance training but you still carry yourself like a sprinter/football player running distance. Just make adjustments from the comments. More relaxation in the shoulders/arms. Try not to cross the center line with your arms. You want them going forward/backward, not side to side.

I'm really surprised you see this as 5:35 pace. The stride is a good deal to long to be someone running in the 5:30s, even for a slower loping stride.

You make 4:35 look easy.
I don't know anything about form or anything, but it seems like your legs swing back more than they swing forward for most people and you're kind of leaning forward. You may be chopping your stride for the treadmill though. And I wouldn't call it a problem, just something I noticed.

You affirm that "you get tight in the upper back during higher paces" so. Your arm carriage is way too high.
Lower them down like a locomotive see (youtube) for examples of the great Norwegian runner Grete Wait's arm carriage. She has a great example in what you should achieve.
However, moves your arms like your doing for higher paces. Running is fine tunning and knowing when to alter your techniques give the race.
For instance, Rafael Nadal would be able to beat the big servers in the game IF HE LOWERED THE TENSION IN HIS TENNIS
racket. Playing with as many rackets with different tensions makes a tennis player the tools to beat the other player. Federer would win more against the big servers if he got used to manipulating the stroke with an even lower tension strung racket and he already uses the lowest tension on the tout. It's just fine tuning. So fine tune your run. People just love my analogies that's why they get better.
Arm carriage is probably high because of the 400 background. Reminds me a little of Pierre Ambroise Bosse the french 800 runner.
Youre quad dominant, can tell that by a slight forward lean and slightly dorsiflexed foot on ground contact. See Centro i.e. Glute/Ham dominant runners usually run straighter up and reach for the ground (Michael Johnson, Nijel Amos)
I wouldn't change that much because your running style is like this for a reason. Only thing is arm carriage, little lower for longer distances, and a better hip extension.

Then I saw this guy on the side of the page. Check his page out. Runs a 4:23 mile with a stroller and does speed work next to a police radar station.....I like this guy.
Jesus, this dude has great form. All you people critiquing it should see me run. It is making me think I wasted 30 years doing a sport I wasn't suited to.
