I'd put Sgt. Pepper's up there.
Street Spirit (Fade Out) on The Bends by Radiohead.
Bob Dylan has had some great ones: Sad Eyed Lady of the Lowlands - Blonde on Blonde, Desolation Row - Highway 61
Definitely agree with Street Spirit, and The Tourist is an excellent close to Ok Computer too. I also like Reservations from Wilco's Yankee Hotel Foxtrot, In The Back Seat from The Arcade Fire's Funeral, and Leif Erikson from Interpol's Turn On The Bright Lights. When The Levee Breaks from Zeppelin IV and Echoes from Pink Floyd's Meddle (even though it's longer than the entire rest of the record) are both classics. The greatest in my opinion though is The Who's Won't Get Fooled Again from Who's Next.

I've always said that Bright Eyes really knows how to close out an album:
"Let's not Shit Ourselves" from Lifted
"Road to Joy" from Wide Awake
Both pretty heavy finishes
But then I heard "Lime Tree" from Cassadaga, and it's just the line, "I felt lost and found with every step I took..." then silence, hardly even a cadence. It just hangs there. Powerful shit.
life in a glass house - radiohead
Brain Damage/Eclipse
There is no dark side of the moon really. As a matter of fact, it is all dark.
This is an easy one:
"Jungleland" off of Born to Run - Bruce Springsteen
"Salt of the Earth" on "Beggar's Banquet"
Considering it was their last recording together, I would say "The End" followed by "Her Majesty" on the Beatles Abbey Road album.
I second Jungleland. There could be no better ending to that album. Not even Born to Run, itself, would be a better ending to that album.
Jimi Hendrix: 'Bold as Love' on the Axis: Bold as Love album
"Last Train to Clarksville" on 'We are the Monkees.' A classic!
Hurt from NIN's Downward Spiral
Buckets of rain - Blood on the tracks
Bob Dylan
I'll give a third nomination to "Street Spirit" on The Bends. And offer these additions:
I Didn't Understand - from XO (Elliott Smith)
Dream a Highway - from Time the Revalator (Gillian Welch)
Cradle to the Angel - from Bloomed (Richard Buckner)
Easter Time - from The Beekeeper (Tori Amos)
and possibly my all time favorite......
For You - from Tracy Chapman's self-titled debut
My 1st song from The Black Album-Jay Z!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!! seriously
