Not sure the purpose of this post but you should see Sifan Hassan winning London last year after training all through Ramadan.
I think many avoid racing during Ramadan. It's only one month long, pretty easy to not choose a race during that time period. A better question would be how do they train during it. For example if there are any Ethiopian Muslims headlining the London Marathon this weekend. Some of their biggest weeks of training would have been during Ramadan. I wonder if they have a higher injury rate.
I don't believe they don't cheat.

you don't have to do Ramadan strictly. They won't throw you in jail. If you have some good reason not to, like training for a race, they won't even frown on it.
Sifan did not race London during Ramadan, that's a big difference.

Have a couple muslim friends, and they will usually train early in the AM after eating and later in the evening so that they can drink during the workout + eat directly after. None of them race during ramadan.
There's quite a lot of Muslim sports people especially in soccer and they all seem to arrange their training appropriately but come race day or match day, they will break fast to ensure they perform properly.
As others have said, Ramadan isn't a strictly adhered to practice. If there's reason to break fast if you are ill or have a physical job then it's fine.
Also Ramadan is at different times of the civic year so if it falls in December the fast is relatively short as it's from sunrise to sunset. If it occurs in June it's a different story.
