The best analogy for running heart risk is a ski jump. At the top of the jump (sedentary) risk is highest. With 12-30mpw, risk reaches its lowest point. That's the low point of the ski jump. As runners go over 30mpw, risk goes up again like with the takeoff point of a ski jump. Yes, the risk is higher than at 12mpw, but it's WAY LOWER than for your sedentary peers.
Still, you only have one heart. Older (50+) runners shouldn't take heart health for granted just because you've never had any chest pains. Arterial blockage can reach a shockingly high level before it results in symptoms.
I train extremely hard and I train alone so I see a cardiologist twice a year and get a full battery of heart tests once a year. One of the tests showed a loss of arterial elasticity so we're treating it with Vascanox, which is mostly beet root extract. Arterial elasticity has improved.
