Chainsaw accident - not when it happened, but when a paramedic dumped antiseptic in it and scrubbed it down, then later a doctor stuck needle directly into the wound to numb it.
But for longevity- I burned my forearm on a dryer vent, which melted my flesh. That hurt like hell for several days and I barely slept. Left a hell of a scar.
Most serious accidents don't hurt much when they happen, it's later that the pain kicks in.
sciatic nerve issues. Intense shooting pain at random moments. It happened once while I was driving and I literally screamed as loud as I could while tears streamed down my face until I finally had an opportunity to pull over and masssage/straighten my leg.

when i was in college the team doc gave me a cortisone shot for the inflammation in my posterior tib, to get me through the season.
stupid thing, but i did a workout with the team and then ran right over to the office to get the shot (leg was nice and painful at that point, lol). doc had 2 student AT's in the room, and he put the needle in my leg, moved it around a bit while injecting... wound up hitting my bone (where the inflammation/pain presents the most) and I damn near hit the ceiling.
Ridiculous intense searing pain, albeit for just a couple of seconds.
Last summer I accidentally cut my wrist (non-palm side) with a kitchen knife while stupidly trying to cut the lid of a plastic bucket. The knife slipped and cut into my wrist. Blood was spurting all over the wall like you see in the movies...a big splash of blood would come out with each heartbeat. I just missed the tendons so I didn't lose any finger functionality (doc said it was close).

ER doc prescribes oxycodone for four broken ribs I got in a car wreck. Forget that I'm allergic to the sh*t and I'm sick within a few minutes of swallowing the pill (this was next day after my wreck). Driving in pouring rain at night, I get violently nauseous. Pull over on side of the interstate and throw up multiple times, broken ribs grinding together with every heave. Throwing up feels bad enough, but with freshly broken ribs? God almighty. Thought I was going to die. Beyond excruciating.
Kidney. Stone.
When I was 16 I had a green stick fracture of both bones in my forearm. The doctor gave me a huge shot in the armpit to numb it so he could finish breaking the bones. He did it with his bare hands. I don't think the anesthetic did a thing. I will never forget the sound of those bones crackling like that. Still makes me woozy to think about it.
When I realized I'm not enough.

Tib/fib break. Not when it happened, but riding in the ambulance when the bumps and turns would move my leg around. I asked the paramedic how long it took for the morphine to kick in and he looked surprised and said immediately. I explained that it made any difference at all. So he gave me two and half times the dosage that he originally gave me. Interestingly enough, that may be the best I've ever felt.
