You build a mental edge and like somebody pointed out, all of a sudden 100 miles isn't much when every run is at least 25 miles. It comes down to most people not wanting to put in that kind of time.
How many people can train at that level without getting injured? Keep in mind that a lot of this is trail running at altitude, so it's even more stress on the body than 180 on the roads would be.
