Your body just doesn't recover as well at altitude. The thinner air makes it harder for bones, muscles, ligaments, etc, to repair themselves. Being in a more oxygen rich environment makes your body recover quicker.
I'm not sure how much of an effect being at 5,000 feet would have on injury, I think in ritz's case it was more training surfaces....alot of the places he ran in bolder were hard packed dirt/rock roads that weren't too forgiving to someone who is prone to stress fractures. He mentions the wood chip layered trails in oregon as being much easier on him.
Ritz has an eating disorder - it's nutritional.
Do you hear of mountaineers with stress fractures? Are guys coming back from Everest, K2, Denali, etc riddled with stress fractures? The answer is "no".
Altitude negatively effects absorbtion rates. For most people it is not enough to be a factor. For some it is.

they aren't trying to run 120mpw on everest though.
Did you get that from the Hudson School of Physiology?Most of the "absorption" studies were done at simulated altitudes of 5000m or 16,000+ft.Ritz has eating "issues" to which he attested to after the WCs in Japan.
