Ugh, iTunes is terrible. Personally I use Winamp. That way I have my entire MP3 library that I've downloaded from various sources, then just right-click them and add them to my Shuffle. It's incredibly simple and I think WAY better than using iTunes.
So how do you get Napster,etc MP3s to work with iTunes? Don't use it. There are plenty of other audio managers out there that work with iPods that are simpler and better to use.
... As for the ear buds issue, I hate them. I just got a pair of over-the-head headphones that don't budge when I run in them. Work well for me!
