There are probably some better answers, but Oklahoma State's home course is probably the most difficult courses I've ever run. The hills are really no joke, didn't help I only experienced a 10k on it so maybe that made it feel more difficult. Second one I might say is Griak in Minnesota. The constant rolling hills make it tough to get in a rhythm, but the biggest thing for this course is it an early September race and it can be really hot some years and if you are one of the last races of the day it can be pretty ripped up and muddy.
I've heard some people say Kansas's Rim Rock is difficult after they changed the course or something? I haven't ran to confirm or deny this. Iowa state's home course is moderate, but I definitely wouldn't call it a difficult one. I also remember watching D2 nationals a few years back at Pittsburgh's Schenley Park. The times were really really slow, 33:30 was All-American. From what I remember that race was hilly, but the footing was absolute slop and the weather was rough, so that course was probably pretty tough too.
There are probably some random smaller races here or there that are really difficult, but these are the ones I've ran or heard from other people about being difficult or slow courses.
I've run the Oklahoma State course, Griak, and Rim Rock. In order they are Rim Rock, Griak, Oklahoma State. Rim Rock has a few hills, both up and down, unlike anything outside of some crazy high school courses I ran, and the footing can be questionable at times. Griak is also iffy at times, mostly because of weather.
Oklahoma State's course is just a road course that happens to be on grass.

The old IU course was the hardest in the past 50 years, no one broke 29 for six miles or 30 for 10 km on it.
The six mile record was held by Craig Virgin and the 10 km by David Kimani.
Schenley Park is harder than IU, though initially, I was only thinking D1.
