From listening to the Sweat Elite podcast I don’t think this was supposed to be an all out effort.
Nope. Look at it like a road 5K in the middle of heavy training (did you see his 5 x mile vid from a couple weeks ago?) at the start of heat and humidity acclimation with no prior racing in months. Since he and his coach weren't talking about an injury issue in that video and he's been doing monster work in warm weather it's safe to assume it was a lack of sharpness and the weather that kept him from running any faster.
I don't think so at all.
Why should we be?
If he was running to win which he did, then he achieved his goal.
And had a good workout to boot at 2:05:30 marathon pace.

Depends on what you mean. He wasn't going to medal and still isn't, so nothing has changed there. So, I'm not sure what your concern is.
I think 14:13 per 5K pace is closer to a 2:00 marathon

For what its worth, in this fairly recent podcast, Lee Troop said Jake is coming off a hamstring injury and has just resumed serious training.

Workout. He is already in. Everything should be towards performing in Tokyo.
Really rooting for Jake - seems like every Olympic cycle there's a guy or two who gets that cinderella experience (Brian Sell, Ian Dobson, etc.) and Jake fits that bill for Tokyo.
This result is in an off distance 2 months out from August 8 so doesn't really mean anything.

Fun fact: he ran 14:13 in his last 5km race, almost 5 years ago - to the day.

He was pacing someone who dropped out, so he slowed down to pace the other finisher.

Even if there was a cause for concern by someone, it wouldn't be "we" unless we are Jake or the US Olympic officials.
And whether that is a good result or bad result depends on the purpose of the run and his current training schedule. If this was to be an all out effort after a taper with the race on the near horizon, Jake should be concerned. But those circumstances aren't present.
Why even pay attention to such things?
This is why the US sucks at the marathon. We're making excuses for a terrible performance and we think a 2:12 is AMAZING.

Who has said 2:12 is AMAZING in the last few years, with regard to US marathoning? Outside of a few debuts, I don't recall anyone being astounded by a 2:12. 2018 and before, 2:12 was a plateau that a lot of guys were on. Now things have progressed:
16 guys ran under 2:12 in 2019 with 3 sub-2:10s
16 guys ran under 2:12 in 2020 with 8 sub-2:10s
