I can name you 10 ultras right now just in the USA that are tougher than this (12,000 feet of climbing in 62 miles on pristine single track trail is NOT that hard).
In no particular order:
1. The Barkley Marathons
2. Hardrock 100
3. Badwater 135
4. Grindstone 100
5. The JIM (Jarmans Invitational Marathon)
6. Eastern States 100
7. Massanutten 100
8. Zane Grey 50
9. Speedgoat 50K
10. Hellgate 100K
There's no running in the Barkley Marathons. It's not a running race. It's a giant scavenger hunt.
It's true. Sage Canaday has specifically avoided this race due to high DNF and injury rates. Dean Karnazes has said it's one of the toughest things he's ever done.

Twisted Branch is harder.
Pretty bold statement for a first year race with less elevation change than 50ks and a 75% finisher rate for a race that doesn't require a prerequisite to run the race. A guy from Alabama who apparently hasn't run any ultras in the past 5 years won this race. Not quite what I called competitive.
Hootnany 100k? I think my grandson did that marathon last year. He runs a lot and can run a mile in under 5 minutes.

In the typical 100k road race you are carrying absolutely nothing.
Try running a 6:05:00 100k that is hard.
