The treadmill typically takes like 20 seconds to speed up to 12 mph. I am guessing as it is getting up to 10-11 mph before he quits (15-20 seconds) but maybe he made it to 12 mph before he quit. Some treadmills do go past 12 mph.. I have done strides at 13-14 mph on some treadmills before.
Sport is probably Ultimate.

Most of the responses are right on track. There is more than a 95.7% chance that when he actually gets to the treadmill, he realizes that what he said is not actually what he meant because he was wrong either about the pace or the distance or whatever foolish conversion he was doing in his brain.

Ultimate is my guess too. In most other sports the phrase "went to worlds last year" makes no sense. I know some good ultimate players but I doubt most of them can run a 5:00 mile.

time to develop ones cardiovascular system.
Running a mile in 5 minutes is not easy, and while it's easier on a treadmill, it still is moving.
I have no idea who this guy is, but lets assume he's a squash player or something like that, in reasonable shape and not overweight.
And I will assume he's under 45.
Maybe he lasted 7.5 minutes at the most? 10 minutes?

Depends what kind of sport. If the sport requires at least some focus on endurance I would say he made it about 3/4 of a mile. If the sport requires all round power and fitness but not tonnes of endurance I would say he made it between 1-2 minutes.
People make outrageous claims all the time.
I think it would make good for reality TV and betting.
If people would put there money where their mouth is, it would be great for ratings.
If he was so confident, he should be willing to bet money on it and do the challenge on TV.
I'd watch.
The thing is he might do some research before going on TV and back out, realizing he was aloof.
I would bet that the guy can't even run 12 MPH never mind sustain it.

no way this adult guy can run 30 minute/6 miles
but the thing is, there are enough anecdotes about amazing running feats that they can't all be dismissed.
regularly on this site someone asks if soccer players can run a good mile. Inevitably someone says 'yeah my soccer team did a time trial on our track and some dude ran 4:30. Or 4:40 off soccer training. Everyone else ran 5:45 but there is often one guy.
and of course these africans who take up running and a year later are running 3:32 or 12:5x or some such thing.
all these impressive feats can't be ruled out.

There's a treadmill at my church gym that goes pretty slow on the 10 MPH setting so it's not unreasonable that this other treadmill could've deteriorated as well

Quite possibly. I'm going to say about a half mile or 800m, even for someone "relatively fit" it's not that easy to go much further at 12mph.
I have a college golfer in my office who jumps on the treadmill before lifting weights. He claimed to run 6 minute pace from the start. We convinced him to run a road race and he surprised with an 18:30. Sounds like the guy in the post but 5 minute pace is 10x harder than 6 minute pace.
