I'm a 28yo male, former competitive runner, and have been dealing with tibia issues for the last year, despite no running. I was diagnosed with osteoporosis via DEXA scan, but subsequent blood and urine tests haven't found any causal issues. From what I can tell there are not many cases like mine, so thought I'd see what the Letsrun community had to offer in the way of advice.
Timeline:
- Ran up to 90mpw throughout college - plenty of soft tissue injuries, but no bone injuries
- Immediately post college had tibial stress reaction, but clear cause (returned to running too quickly after non-running injury)
- Starting ~3 years post college, now running ~50mpw, suffered three femoral stress reactions over the course of ~ 2 years; maybe strange, but these things happen and I was a bit older, so just treated w/ rest and seemed to get over them
- Last March, decided to take break from running to let soft tissue injuries heal
- Last May, started having SF-like pain in tibia after short hike -- MRI revealed tibial stress reaction and subsequent DEXA scan showed borderline osteoporosis/osteopenia (don't have the scores off hand, but want to say they were around -2.5 to -2.8)
- Referred to endocrinologist, labs (blood, urine, 24 hour urine) all came back fine, decided to just monitor as leg was starting to feel better
- Pretty much pain free from mid July to mid September
- Occasional bouts of pain in tibia from mid-September through January, usually resolve within a few days, didn't return to running, but swimming + core / light lifting for exercise, general minimized walking/standing when having pain
- Consistent tibia pain returned in February, MRI confirmed tibial stress reaction, 2 weeks on crutches, pain first week back on leg -- now having some good days, some bad days
Other Details:
- Taking D3 and Calcium daily
- Diet is good - plenty of fruits/vegetables, meat, dairy, etc. (no dietary issues noted in labs)
- Seeing PT, mainly to talk through pain patterns (no strengthening now as still having symptoms)
- Pain doesn't seem like it's impacted by footwear (dress shoes, racing flats, barefoot, support running shoes, sneakers all feel same)
- Have chatted about medication options for osteoporosis (Forteo, Fosomax, etc.) - consensus has been that I should avoid given side effects / limited research on long term effects on otherwise healthy young males
- Understand that weight bearing exercise is ultimately going to be one of the more effective fixes for the bone density issue, but can't do much of that now with tibia injury
- Working on seeing another endocrinologist to get a second set of eyes on things
Right now, just want to resolve tibia pain so that I can resume my daily life without worrying about walking/standing too much, but obviously there's an underlying medical issue that needs to be addressed.
Has anyone seen anything like this before? Thoughts/advice?
