Lifetime ban for a first offense?
Add blood testing to the urine testing (stuff sticks around in the blood longer typically).
Restitution of winnings for the athlete's career.
(The last one goes against US law so would be problematic.)
since there's really no way for drug tests to keep up with the designer drugs from what I have heard, how about lie detector tests?

Polygraphs aren't very accurate, they've been debunked by the AMA and APA.
The lie detector test is not 100% accurate, it can be fooled or the operator / interrogator may screw up but the kicker - not admissable in court. (Which is where the athlete's appeals will end up.)
The best bet would be to enact national laws (not sports rules) banning PEDs, define earning sports income while cheating as fraud, allow athletes / coaches / care givers / federations to be prosecuted together for racketeering and conspiracy in cases of systematic doping, collect blood, urine, and DNA samples throughout the athlete's career for future testing, and create a 20 year statute of limitations for doping offenses. Oh, doping convictions not only carry a lifetime ban from sports but also require prison time for the fraud or racketeering charges.

banning PEDsAs most PEDs are drugs which are supposed to be used in a theraputic manner, how do you propose to ban them without negatively impacting those people who actually need them?
True. The drugs themselves are not banned but use outside of their intended medical purpose is a crime. Similar to controlled pharmaceuticals now, if a doctor gives me Oxycontin or morphine but I have no demonstrated medical need for it, we are both guilty of improperly using a controlled substance. So when the US Attorney supoenas the defendant's medical records everything had better be in order. If not, then you also get the doctor(s) on charges and the state medical licensing authorities will have pointed questions. Logistically speaking, the USADA detects the substances or markers and turns the evidence over to the legal authorities. No one from USATF, USOC, IAAF, etc. can run interference or spin it once the prosecutor has it.
What you do is create an atmosphere where doping activities and their coverup are either too complicated or expensive to pull off. No doctor, coach, or agent would be willing to take the risk of losing their livelihood and freedom for an athlete with a short earnings window. It would be too exorbitant a cost for the athlete to pay hush money to the involved parties. National federations risk sanction and having their national teams banned from championship competitions if they are found complicit. The bigger the conspiracy is, the harder it is to keep everyone involved silent. Keeping that circle of silence for 20 years would be damned near impossible.
Never work. "Off Label Use" means that any doctor can prescribe any medication for any illness. You go to the doctor, tell him you feel fatigued all the time, he can prescribe EPO or that you are having trouble with sleep apnoea he can give you Provigil or whatever, just make it up, and both of you are safe from the law. Hell, you can tell him that you've got a hangnail he can prescribe you some steroids if he wants.
