I think you go way too fast on your recovery days considering your PR. I think people who are running around 15:00 for the 5k should be doing their recovery days slower than 6:50.
I suspect you've been burned out for a long time. How do you feel in workouts and races? What sort of workouts do you do? Do you vary your mileage? When you say you do ~60mpw, do you ever go down to 40 for a week? (you should every 3rd or 4th week)
Do you have a particular race or group of races that you are focusing on? Find some races that you want to focus on and significantly decrease your mileage to about 30mpw. Start racing once you drop the mileage and stay low for a few weeks and do the rest of the races at low mileage. If you're burned out, you'll probably see a little bit of improvement just resting a bit.
To solve the true burnout problem you need to take a month (or two) completely off from running. And then when you start running again, to avoid burnout make sure you take proper recovery days and recovery weeks (aka down weeks).
