Assuming you are asking about what specialty is best for working with athletes, especially distance runners, it is a sports medicine fellowship or orthopedic surgery.
Med school+family medicine/PMR/Orthopedic surgery residency + sports medicine fellowship.
If you do orthopedic surgery for residency I believe you may be able to just not do a sports medicine fellowship. I'm not sure since I don't want to do a surgical residency.
Be a hematologist/oncologist so you can prescribe yourself EPO analogues and hematopoetic steroids.
Chiropractor or podiatrist.
