I had to take 2 months off completely earlier this year after a serious bicycle accident. My 1500 PR is a 3:55 (and I ran a 3:10 1200 a few months back). When I came back, I did a very hard, although not all out-mile, and it was a 5:16. I don't think I could have broken 5 minutes in that if I'd gone all out. So I'm going to peg you at 5:20 for the mile considering the longer break.
You've got good speed, so I'll give you the sub-60 at a 59 for the 400. 800 meters, 2:15.
