4:06.0hy if the record keeper did their statistics properly. An in depth, race by race following of Lacy's career can be found here:

Steve Lacy, 4:07.4h, McFarland HS (Wi) sr 1974....unsure of the meet
McFarland needs to update their school record board. I would use the mile to 1600 conversions instead of the 1500 to 1600 conversions, but those are listed below too.
The 1600m conversions are in parenthesis:
4:07.6 (4:06.2) Monona Grove Invitational - May 10th, 1974
4:07.4 (4:06.0y) International Prep - Mt. Prospect, Illinois - June 8th, 1974
3:47.2 (4:04.0y) US Junior Olympic Outdoor - Gainesville, Florida - June 14th-16th, 1974
3:48.5 (4:05.4y) Golden West Invitational - June 23rd, 1974
4:13.1 Iowa

Steve Lacy ran a 4:07.4h mile in 1974, and that converts to a 4:06.0hc 1600 meters, and yes I agree if there is a 1600 list or a mile list, then 1600 or mile conversions are appropriate for that list. I would be inclined to have a separate list for 1500 meter performances, and not convert them for a 1600 list or mile list.
I see a y after your 1600-meter conversions and I am not sure why you placed that there. Typically, a y or yd refers to a yard mark (as opposed to a metric mark, denoted by the letter m).
I guess each list maker can make up their own rules and notations, as long as it is clear. I try to follow what I believe to be common practices, but as a compiler of list, I do have my way of compiling and notating list. For example
If I were to make an all time high school 1600 meter list, I would list Lacy's mark this way...
Steve Lacy 4:06.0hc (4:07.4hy)
h=hand
c=conversion
y=yard mark
the () contain the original mark

He had been in the hospital for four days with pneumonia just one month before the meet. Rain before the event so had to stay in check in tent because they wanted everyone to sit checked in so no warmup. Two inches of water on the track when the gun went off. We wanted to use the brooms they had to sweep it off. Of course right after the mile they did just that. He also ran the 880, two mile run and anchored the mile relay with a 48. They did all that in one day back then with the mile relay after the two mile. Backwards Louisiana won't recognize him as the 1600 meter record even with the mile being longer. Last time I checked it was Keith Iovine.
