So you are implying that actors should check all the props they are given to use? Or that they should boycott films where guns are used??
It seems like they shouldn't need to check the gun, there should be a person on scene whose only job is exactly that.
That being said, I cant imagine pointing a prop gun at somebody and pulling the trigger without triple checking it first. My understanding is that most of these are real firearms loaded with blanks, for practical reasons, its easier to source real guns and blank ammo that fits into a real gun.
That being said, it shouldn't be that hard to get a gun Smith to make the gun unable to chamber normal ammunition, for most types of guns. Depending on the type of firearm, it should be possible to shorten the chamber significantly, to where a round with an actual bullet in it has too long of an overall length to feed.
I'm also not sure how you shoot two people at once, I guess a shotgun?
But really these mistakes shouldn't happen. Law enforcement does force on force simulations all the time without shooting each other. We use special chalk rounds and follow specific rules to prevent anybody from accidentally loading real rounds. Film sets should be able to come up with similar rules.
