But, your premise is slightly wrong. Its not altruism, its prosocial duties. There are numerous examples of things in society where an individual takes on a greater risk or negative for the benefit of a harmoniously functioning society.
This is closest to utilitarianism, which is one of the best moral philosophies around.
Not everything altruistic is a "correct" decison from the standpoint of utilitarianism.

Declining to receive the COVID vaccination shares common characteristics with all of these examples of free-riders. The issue could be resolved somewhat by incentives and/or penalties/exclusions. Incentives could include tax breaks, or stimulant payments for those that choose to vaccinate. Alternatively, those that choose not to vaccinate could be excluded from flying, large public events, attending school, etc. Of course, the politics of some of these solutions probably make them unfeasible.

There have been a few thousand cases of people with the vaccine getting infected again. A young person not getting the vaccine is not putting anyone at risk.
