Title says it all. Should I use a certain incline to get accurate fitness? or pretend I'm Nike and have no incline and a running start? I'll try 11.5mph (16:12 5k pace) and perhaps up to 12mph (15:32 5k).

That chart is infamously, but for the low incline you want (1%) it's probably not far off. Cross your fingers that your treadmill is calibrated accurately.

Oh, why is it infamous? And yes, treadmill calibration can be a big issue.
Running on a treadmill can be good training but going all out effort would be a wasted effort.
Where are you going to rub the testosterone during this test? I heard the rectum is very effective AND doesn't trigger tests nearly as easy.
I mean, I hear this from a friend...
For a time trial, you prob want 0% to 1% incline to get the fastest time possible. I wouldn't expect treadmill running to relate exactly 1 to 1 with outside running though.

agreed.
I believe the treadmill should be used for easy runs and slow steady runs only. Anything faster could lead to an injury, because the belt speeding up in reverse really messes with the gait of the runner, and the jerking effect increases.

I agree. Let me pile on to this.
Most treadmills are not even accurate. So if you are doing the time trial to set paces to run on the treadmill then have at it. But I would never recommend using time trial information from an effort on a treadmill.

Glad I'm not the only one who felt like this.
Op, what's the purpose of this anyways? To see where you're at? Run your TT on the track. This way you cut the guess work out of treadmill manipulation or if it's calibrated properly. You get to train to see how well you can pace yourself rather than some belt making you run a certain speed. You know openly you're probably not in peak fitness so some variation may occur due to weather or just not feeling it that day. Besides, where's the pride in doing this on the treadmill in ideal conditions? You're gonna tell people you just rocked a sub 16 5k TT on the treadmill? Seriously bro....

Have to say, I agree with all of this. I usually try not to ask "why" or "what's the purpose" when replying to threads, but all of this is true. Treadmill running should be used very sparingly.
I do workouts all the time on the treadmill and ive found them to be an accurate gauge of fitness, at least on my treadmill. I took it with a grain of salt until races backed up what the treadmill workouts said

you don't have to simply cross your fingers...
you can actually check.
