I lived in Japan for three years and it's really easy to travel around the country. Their train system is top notch and many of the signs are in English. Most Japanese people understand English, but may not speak it well. I found that if I wrote something down most people could read it and help me out. The Japanese people are extremely friendly and helpful. You could easily spend a few days in Tokyo and then take the Shinkansen Bullet train down south to Osaka/Kobe/Hiroshima area.
Make sure to watch Gaki no Tsukai, the greatest show on television.

Language barrier is not a problem. Japanese are amazingly helpful to tourists. Get a JR pass, and you can travel around for cheap.
Very easy to get around. I was there earlier this year for ~10 days and it was great. A lot of signs are in English, their public transportation is very efficient, and the people are very friendly.
Fantastic food, go to the fish market in the early AM and enjoy the best sushi you'll ever taste.

I dunno, I think the language barrier is a real issue even in big cities. I've been to over 30 countries and Japan and China were probably the toughest I've encountered if you're relying on only English. I guess there is enough on signs to get around, but with people at many restaurants or convenience stores there is basically no practical level of English.
Go to Okinawa, beautiful but nobody speaks or understands English except for yanks and teenagers.
