Treadmill running, when done right, is actually superior to running outside. Just make sure that you do the bulk of your running on a 3 degree incline. This significantly reduces the impact of running on the joints, while at the same time providing a greater challenge to the muscles.

I would worry about 1.5% because it's getting close to flat and the impact on the joint goes up. But to answer your question, there's nothing magic about 3%. It is just a nice point where (A) the impact on the joints is lower, (B) the training effect on the muscles is greater, and (C) your running form at 3% is close enough to your running form on flat ground so that your treadmill running will transfer.

It depends.
Personally, I use 2% and it seems to mimic the effect of outdoor running.
