Weird. Dates are for getting to know people and deciding if you want to see them again, not committing to some contract that has to be observed.
Anyway, back to the main issue. When you asked her out and she said yes, what did you ask her out to? Otherwise its likely she thinks you played her and is now avoiding you out of embarrassment at being played. You asked her out, she said yes, expected you to get back to her with a date, place and time, and nothing.
What did you expect to happen? Did you suggest meeting up at all? Are you a child?

dude the fact that you needed to tell the board you had a dying parent, shows you have bigger issues to resolve. You need to leave that behind, if you cant, then you can't be looking for no relationship. Forget her, or you will become a stalker, if she wants you, she will come looking for you. If she doesn't then is all good, there are ~150 million women in the US. Get in shape, look hot, and you will find plenty of other female companions. And yes, she could be playing games, women are empowered to be just as free and sexual as men always been.

Don't do it. Forget her. Move on. She is not interested. If you are having this much trouble with it, what happens when you do go out and she dumps you. How awkward will that be. Doesn't sound like you could handle it given this confusion, and it is not good for the work place. Find a life outside of work, you'll be happier. All of your conversations will not be about work, those you work with, or the industry your in. You'll be better off in the long run. Plenty of fish outside of your little pond.

I had to learn the hard way not to date co-workers, but that doesn't mean it can't or won't work out for you. Life's too short not to go for what you want.
