The key is to set the rule in advance: "This year we are making our own gifts for each other" or "This year X dollars needs to be the spending limit on our gifts to each other." Explain why. Don't let it be a surprise, and establish that she won't get you something expensive either.

This is the last thing to buy her. She knows she is pregnant. This would just remind her. It would be up there with getting her a vacuum. Could always do a night away or a spa day for her. If that is too much get her a photo album for her to fill in. Put a ultrasound picture on page 1. (Even better if the picture is of your kid)
