Hey,

I’ve heard conflicting information. Given my lesion was on the medial femoral condyle (weight bearing); 2 doctors told me to stop running. The surgeon I finally used told me not to give up and to do the PT and see how it goes.

I am now a bit over 19 months post op. I still have occasional pain in the knee but NEVER like it was pre op.

I have done some marathon race pace at 7-7:15 min miles up to 14 miles. Knee usually feels good the following day but then tight and sore about 48 hours after.

I have been able to maintain about 25-40 miles a week running every other day; sometimes consecutive days.

I just completed a 50 mile ultramarathon on gnarly mountain bike trails.

It took me a tad over 13 hours but I got a belt buckle!!

This was my first ultramarathon. It seems if I run slower; I’m good. The faster stuff seems to aggravate it. I did a 5K in 19 mins and was sore for 2 days which was about the same after the 50 miler.

I hope to get the knee used to a steady 7:10 pace as to requalify for Boston.

So; I’ve been pretty damn happy with my results and hopefully it continues to improve.

BTW I’m 42; 6ft 183 lbs.

I'll add my update. Had surgery five and a half years ago. Cartilage lesion was on the medial femoral condyle. Two doctors told me I'd never run again. So I went to doctor # 3, and he said let's try microfracture and see how it goes.

5+ years later, and I'm still running 30-40 miles a week. I've done countless 5 and 10ks, and a handful of half marathons. The knee feels good most of the time. After a run of over 10 miles, the knee definitely talks to me for a couple of days afterwards, so I minimize longer runs. I'm probably done with marathons, and I'm okay with that. I'm cool with 5 and 10ks, and maybe a half marathon every year or two.

After having two docs tell me I was finished, I'd call that a big success.

My surgeon told me not to give up!

Oct 13th this year (14 months after surgery) I ran the White Water Center 50 miler. It was very slow but I did finish before the cutoff and got a belt buckle.

Ran a half marathon at marathon goal pace (7:12) 3 days ago. Knee was a bit sore and cranky for 24 hours. But I’m going to try a marathon Feb 16th. Anything under 3:30 will be a win. I would love to qualify with a 3:09 but that’s likely a pipe dream.

Gonna give it all I have.

Any reason why these microfracture procedures don't retore the joint to full functional capicity as well as a better long term outcome? The study posted on this page from Solheim et al published in "Knee Surg Sports Traumatol Arthrosc." doesn't sound very optimistic for a long-term prognosis.

I recall seeing a lot of pro athletes (footbal, basketball, skiing, etc.) getting the procedure and coming back at or near pre-injury level. Then they retire in a few years or so and we never hear how the joint is holding up in their retirement or whether or not they eventually needed joint replacement. So, perhaps for pro athletes it's a short-term fix to get the athlete back out there at near full recovery for a few more years, while the long-term outcome several years down the road isn't that promising.

I would suggest looking into DeNovo NT by Zimmer or Biocartilage by Arthrex as opposed to microfracture. Another option, if there is bone damage as well, is an osteochondral allograft, which can be combined with the other two if need be.
