Use a file to grind flats on two sides, then use ViceGrips or pliers to get them out. ViceGrips are way easier than regular pliers though.
The best way that I have had success with getting out spikes that have been worn completely flat is drilling a hole in the middle and then driving a screw into them which turns them the opposite direction. I've never found a spike that I couldn't remove this way. You have to be careful you don't break the drill bit when you are doing this though.
If you can't vise-grip them out, use a dremel tool to cut a groove and remove the spike with a flat-head screwdriver.
