I've seen some threads on here briefly talking about ESWT treatment for Insertional Achilles Tendonitis but since I started treatment yesterday, I figured I'd make a thread documenting my experience for those who are considering the treatment. Studies show that ESWT combined with eccentric exercises has proven to be very successful so I will document what eccentric exercises that I have incorporated into my daily routine as well.
Some background on myself: Early 20's, ran in college, and I have been dealing with insertional achilles tendonitis since the spring of 2021. Took 4 months completely off from running in the summer/fall of 2021 and didn't see any sort of improvement in my achilles. I did PT in late 2021 and the beginning of 2022 and saw a bit of improvement as I was able to build up to about 40 mpw but unfortunately the pain returned. Since then I've been running 3-4 times a week or whenever my achilles feels good enough to run on. Just short runs to get outside and get the blood flowing but I want to eventually be able to compete again. I'm a competitive person and I love to race. I was finally able to see a great doctor in my area who works with athletes and they did an ultrasound on my achilles and recommended ESWT treatment.
I had my first treatment yesterday and the procedure took around 20 minutes from start to finish. Prior to the ESWT treatment, the doctor had me do a few calf raises and a single leg squat on the leg with the insertional achilles tendonitis. The calf raises were a bit painful and I could barely do a single leg squat without being in a ton of pain. As for the ESWT session itself, it was a bit painful at some points but overall was not too bad. I felt some sharp pain specifically in the area where the achilles attaches to the heel but I expected that. Should be noted though that the pain of the treatment is not as bad as the pain from insertional achilles tendonitis.
Immediately after the procedure, the doctor asked me to do some calf raises and attempt the single leg squat on the affected leg. I was able to do both of them without any pain! A few hours after the procedure, my achilles felt a bit sore like it normally does but
I woke up today and my achilles was stiff but definitely warmed up considerably faster than before ESWT. I don't feel much pain while walking around the house which is a good sign so far. I am going to be religious about calf raises, weighted calf raises, and single leg calf raises going forward too. I am going to physical therapy tomorrow and will be getting my second ESWT session done in the beginning of January and I will update my progress then.
