At least Koko has successfully arrived (as of instagram). And the rules are not according to citizenship but to residence. Even as a German citizen coming from the US she will need documented negative test results and the same goes for Brazier and the US citizens in the group.

That depends on both country and individual airport (and I want to say at airports like London Heathrow, it can even vary based on the individual airline you're flying or terminal(s) being used with your connection) Most of the Schengen zone (European common movement area)airports are well set up for transit passengers- I've done a non-Schengen to non-Schengen connection at Amsterdam Schiphol that was materially no different than a domestic connection ATL or DFW.
But many entire countries like the USA do not do sterile transit zones. It makes sense that the USA does not because you would be talking a multibillion dollar investment in transit zones for something like 1% of passengers who are international to international and end up with an additional problem of irregular operations getting a passenger with no right to enter the USA getting stuck in a place without a timely onward flight to their final non-USA destination. I've seen Grand Rapids get used as a diversion point for transatlantic flights when Chicago is under tornado warnings. It's technically an international airport for general aviation reasons even though they have no scheduled international flights and they can round up the staff to process a big bird or two into the country correctly. But you're talking 12-18 hours to get another flight crew into Grand Rapids to get the plane over to Chicago (original pilots and flight attendants would have run out of allowed flight hours so not legal to use them on the bunny hop) and what do you do with the people on the plane who don't have permission to enter the country in the meantime?
As for Julian's group, while there is no magic athlete visa that will get you on a plane right now, countries generally do have exceptions lists for entry. IIRC, Germany even early on was allowing emergency entry for very specific specialists in things like medical and engineering fields if an employer could state there was no one within Germany with equivalent skills that could do the job.
Deadly if your 80 years old or have one of known chronic health issues. When physicians are saying don’t wear a mask unless you have a cough, you most likely do not have covid unless cough and temp 100 or higher. Even then get tested for flu as well. 99% of covid cases have temp. Those with chronic health issues wear a mask. This mess both globally and in the usa is driven by the 90% liberal media and democratic party feeding the media by the spoonful. Is there a virus issue, yes. Undoubtedly. The numbers have been so mismanaged from day one and the number of people being told they are positive who never even had a test, or those who were told positive then an hour later negative. Tip of the proverbial iceberg.
It’s not that big of a deal. Monaco DL got special permission from government entities to host the meet and bring in foreigners. It’s not just Americans and it’s not just American athletes from Julian’s group. The USA doesn’t care if you leave. They don’t have restrictions on travel, but you can’t get anywhere with a USA passport these days. Those sound like travel restrictions but that’s only because other countries won’t let Americans in, not because the USA won’t let people out. You just need to have an invitation and a promise to be permitted at the receiving end. In this case, all foreign athletes competing (likely everyone) have an invitation and permission to travel. Same goes with many European jobs and visa-versa. Commerce is trying to continue but tourism is being curbed purposefully.
