As long as you are completely underwater, it should be really beneficial.
I believe that they're extremely effective as cross training. I usually aim for Class V rapids or higher. They really help build the upper body.

My PT has an underwater treadmill in a whirlpool imitating 'upstream' currents. I think it helps people relearning how to walk. It felt mostly weird to me.
