I don't think that is related to a specific mileage. It has more to do with your bones.
Possibly. Would depend on the following:
(1) did you go from running 0 mpw to running 25 all at once? Did you ramp up in a short period of time?
(2) Do you have a history of bone stress injuries or underlying factors?
I got a stress fracture at 25mpw, but I was coming back from a different one and ramped up too quickly - my body wasn't used to the impact.
Bones take awhile to adapt, and there's a lag. If you've been running 25mpw consistently for years, then probably highly unlikely its a stress fracture. But if you are a new runner, ramped up recently to that, or have something affecting your bones, then it's possible.

Absolutely. I got one running around 30 mpw.
Definitely. Got a stress reaction at maybe 30mpw because I ran a whole lot on sidewalk concrete, the worst kind of surface.
