Yes. Testosterone is that important. It's hard to prove clinically because racing is a major route out of poverty for East Africans who would potentially lose their eligibility if they were found to have DSDs.
On the non-clinical level, however, it's obvious to me that significantly higher T-levels can convey a significant competitive advantage, although not necessarily the same way for every DSD person.
In Semenya's case, it's physically visible. She's tall, broad shouldered, narrow hipped, and has more musculature than her competitors. But there's more. Higher testosterone also conveys the ability to recover faster so these athletes can train harder.
The IAAF testosterone standard, which is about 5 times higher than for non-DSD women, seems reasonable. If this isn't addressed now, countries like China will start testing for DSDs at pre-school level and put these kids into Olympic training programs. In 15 years, non-DSD women will be lucky to make the finals.
