I have updated the list of fastest mile run in each US state to include as many as I could of those states where sub-4 has not yet been achieved. for some of these states there is limited information available, particularly in Hawaii, Nevada, and Vermont, so I am prepared to be wrong about these and this list should be thought of as the start place for a conversation rather than a definitive list.
please note that this list is of 1 mile races by all-comers on state soil. where an * follows an entry on the list, please read the notes at the bottom.

Colorado: La Junta, CO is at an elevation 4,078 ft. not relevant to this thread, but from the department of other cool stuff - in August 2013 Scott Dahlberg, an assistant cross country coach at Colorado State University, won a 1 mile race on a six-laps-per-mile circuit on the summit of Pikes Peak, Colorado, at an elevation of 14,115 feet, finishing in 4:46. Dahlberg is now the assistant distance coach at Wyoming under head coach Brian Berryhill.
Delaware: Vic Zwolak was NCAA steeplechase champion in 1963 and 1964. he went to the 1964 Olympic games in Tokyo where he ran the 3000m steeplechase, and he currently holds the Delaware state record for 5000m in the 70-74 age group. the Bring back the Mile website report him as having run the fastest mile in Delaware, but LetsRunner Blue Hen claims a time of 4:02.0i for "Morgan Moser" in 1972 (there was a link but it no longer leads to anything). I think he means Morgan Mosser of West Virginia University who was 6th in the NCAA mile in 1972. I found a report in the Morning News of Wilmington, Delaware dated Monday, 28 February 1972 - Page 23, that appears to confirm this but it's behind a paywall and I can't see the whole thing. I think the meet was at the Delaware Field House. confirmation of this would be appreciated.
Hawaii: HS boys currently run 1500m but back in the day they used to run 1600m and the state meet record for that is 4:09.5 by Kurt Mench (Castle H/S) from 1970. Hawaii university have not had a men's track team for some time, and the current "Track & Field History & Records" pages on their website don't list any men's performances or records at all, except to mention that Terry Albritton was US Champion and WR holder in the shot in 1976. a LetsRunner mentioned that he thought Duncan MacDonald had run 4:04 for the mile, which seems reasonable because as a master's runner he set AR for the mile in New York, but further details of miles in Hawaii are proving hard to pin down. I so far have zero miles actually in Hawaii.
Idaho: there is some confusion about the exact time run by Jacinto Navarette at Moscow, ID on 13 Feb 1987. Peter Larsson's all-time list (link below) gives him 3:59.75i, but the iaaf website (link below) gives him 3:59.87i, a time also given on the T&FN sub-4 register. I am unable to locate either the original result or a contemporary newspaper or magazine report to resolve the issue. the relevant local newspaper is probably the Moscow-Pullman Daily News but they don't allow access to Europeans because, they say, data protection reasons...
Nevada: at the HS level boys in NV run 1600m not 1 mile so there probably isn't a state HS record for the mile. according to Athletic.net the state record for the 1600m is 4:38.36 by Alex Bledsoe in 2010. at a college level, outside the standard NCAA meets they have the Randall Cunningham Track Classic and a Spring Invitational, neither of which has a 1 mile race. Nevadan Josh Thompson has been under 4 minutes three times, twice in Seattle and once at College Station, Texas, but he has so far not done the deed in Nevada itself. meanwhile, Googling, "fastest mile run in Nevada" gets you 4 million results on world speed records and a thing called the Nevada Open Road Challenge, which turns out to be a 90-mile race for motor vehicles. Nevada is currently zero for miles.
New Hampshire: 4:00.9i Jim Peterson (Georgetown) and Jack McDonald (Greater Boston TC) both at Bicentennial Relays Darmouth, NH 10 Apr 1976. Peterson was 1st, McDonald 2nd but they were given the same time.
North Dakota: a sub-4 mile has been run indoors at Fargo, ND, but details of any outdoor miles in the state are hard to come by. the only "fact" I have at this point is that the track record for Cushman Field in Grand Fork, ND is 4:16.74 set by Nate Peterson in the See Dick Run elite mile in June 2016. See Dick Run is a running shoe store that both sponsors and organises an annual 1 mile festival in Grand Fork, ND.
Oklahoma: the night Wilson Waigwa ran the first sub-4 mile in Oklahoma history, Dwight Stones broke the WR for the high jump at the same meet, which does tend to take precendence if you are searching for this in local newspapers.
Tennessee: the East Tennessee State University indoor track at Johnson City, TN is oversize.
Texas: the indoor track in the Houston Astrodome where Steve Scott set the Texas record was oversize.
Vermont: 4:12.74i Jack Pinho (Springfield) Virtue Field House Middlebury, VT 17 Feb 2018. this is the Virtue Field House facility record. the previous record was 4:18.25i Kevin Serrao (Middlebury College) Virtue House Middlebury, VT 28 Jan 2017. the only other contender I know of is 4:09.82i Bo Parker (Williams College) 1983. this is the meet record for the New England Div III Men's Indoor T&F Championship, but I have been unable to find a venue for the 1983 edition so this might not be in VT at all. Pinho has run faster, he ran 4:07.89 in one of those Last Chance meets in February 2018 but that was in Staten Island, NY, not VT.
Wyoming Gillette, WY is at an elevation of 4,554ft.
