I was in a race with Mark Carroll when he ran 3:57.78i at the University of Florida. Here is the track record to prove it. Supplants the 3:58.8h listed for state of Florida -

I know Webb ran 3:51 at Drake, pretty sure that's the fastest in IA.
I know it is the drake relays record, and I would be stunned if anyone has run a faster mile in iowa at a different track. I was there that day (and had the wicked sunburn to prove it), and it was truly an incredible run.

That leaves out Alabama, Alaska, Colorado, Delaware, Hawaii, Mississippi, Montana, Nevada, New Hampshire, South Dakota, Vermont, Wisconsin, Wyoming, and either South Carolina or West Virgina (since I can't find any record of which Charleston Waigwa ran 3:58 at.)
No way did Waigwa run 3:58 in Charleston SC in June. Alan Webb was first person under 4 on SC soil at Furman Blue Shoes Mile in '07 in 3:57.83. Hammond has since surpassed this time
How about each states fastest miler? I think that would be an interesting list.

Pat Casey definitely broke 4 before the conversion at Montana State, I remember because he got a crazy NCAA leading conversion but the raw time was still 359.
