This is true. And ironically, 401K money is 100% taxable when cashed out by you or your heirs. Every dollar in a 401K account is worth less than a dollar kept outside a 401K.
I don't think so. If you pass on an IRA to your heirs, it becomes a beneficiary IRA and as such, the cost basis is figured as starting from the day you inherited it, and aappreciationciation up to that time is dismissed (not taxed).
