Many star UO runners have been DQed for lane violations over the past five years. Sorry I haven't kept a list, but I go to most of their home meets, half of their conference meets, and all of the NCAA Outdoors. The Ducks often get flagged at the higher level meets by officials from outside the OTC.
I think that the UO coaches must be teaching them to run on the line, or at the very least are failing to teach them to stay off the line.

There should be exceptions in cases like this. If the purpose of heats is to have the best women advance, she should be allowed in. Some people take the rules a little to much at face value, like when they dq'd a girl in the 3 or 5k for a false start. Some level of common sense needs to be applied in some cases. I get "rules are rules" but that isn't what the sport is about. People should understand why the rules are there and DQ people when there is good reason, not just because that's what is written down in the rulebook.
The purpose of heats is not to have the best runners advance. The purpose of heats is to get to the top 8 runners who complete the heats legally to the finals. Otherwise, only 8 runners should qualify for the championships, not 16.
It's unfortunate but she stepped on the inside line on a curve and that is an automatic DQ as it should be

She was all over the lane line on the first part of the first turn, which did not even make the pictures. I thought it very likely that they would flag it. However, calling it without video is a little tough because in the 200 they go by so fast it really is a bit tough to be certain.
The first time I caught a lane violation (it was a conference championship and my first time officiating lane violations) I was able to clearly identify the violation but by then the runners were way down the track and I could not be certain of which runner it was. You have to practice that skill; the access to video is really useful.

American record is 21.87 by Merlene Ottey. It might be dirty but I believe it is official. USA is easier to write than America if one means that particular country.

Not quite. There's an area record known as the NACAC (North American, Central American and Caribbean Association) record, and as such, Ottey's 21.87 counts as a NACAC, Jamaican and world indoor record. Gwen Torrence holds the USATF indoor record (22.33) or just simply the American record.
