Has anyone ever run under four minutes in the mile and not gotten into DI indoor nationals? It came very close to happening last year with Harasyn being the last one in with his 3:59.97.

No. Last year was the first time ever that all runners were under 4 minutes. The second fastest field was 2009 when the last qualifier was 4:00.11

Are these the adjusted times? I think that you could run a 3:59 at UW and lose out to a 4:01 at UW, where the first one is the 307m version out west and the latter one is on the 200m flat track in Wisconsin.
