From what I gather, the people reporting that she died were going off of information from people at the scene, not from doctors. Those who saw a bullet go completely through her head (from point blank range) just assumed there was no way she would survive.
In the media biz there is a saying "if it bleeds, it leads" which proves their sensationalism and willingness to promote it. Notice how when there are reports on Yahoo news, for instance, so often the numbers dead are often much higher than the facts later are. Remember 9/11, then news media was saying "there are 50,000 who work in those towers. We will see tens of thousands dead!"
Tens of thousands, 3,000 but who's counting?...

I don't think most editors thought it was necessary to sensationalize this story in order for it to lead.
I think that the "more important to be first than to be right" mentality is probably what caused it.
The initial reports of her death came both from eyewitness reports and from the sheriff. Not many people survive a bullet going through their head, so it wasn't a far-fetched conclusion.
Once that was picked up by the national media the staff as the hospital released a statement that she was alive and asked the national media to retract their earlier statement.
Watching some of the statements on the live KOLD-TV video stream it was clear there was a lot of confusion about the number of people shot, number dead, and who was dead. Again, no too surprising under the circumstances.
A question here.... was the last Congressperson to die violently the guy that got offed by Jimmy Jones of Kool Ade fame ? I can't remember anyone else.

I don't get the mentality, though. I will much frequently visit sites who are accurate than those who rush to be first with some 3rd cousin of the truth. CNN is the absolute worst. I'll never go to that bullshit website or turn that station on.
What was interesting to me was how fast Wikipedia listed her death date, then as soon as the news organizations retracted the death report, it was off Wiki.
They weren't trying to sensationalize anything. Even if I didn't know anything else about the story, I would assume that when the gun goes off, this man dies:

I think people assumed the same with this story.
If it was CNN or MSBNC, they were misinformed and the mistake was due to someone else's ignorance. If it was Fox News, then of course they are a bunch of liars and are doing this to fan the flames of hatred. That's the correct answer.
