Nope, no other countries speak English. America invented it. Everyone else speaks Spanish, Jewish or Asian.
Your question is well founded. Incidentally, no one speaks "Jewish", smartguy. Anyway, back to the subject: you will find that English is a universal language. However, you can't just go to another country and practice medicine, just as doctors from other countries can't just show up here and operate or practice medicine. In many countries, you might even have to take an exam in that indigenous language. Further, if you show up in japan expecting everyone to speak engrish, you're going to be lost. Your goal is highly respectful. Immerse yourself in the language and educate yourself as much as possible on your proposed country's culture, and go for it! Worst comes to worst, get your MD in the states, and then go practice medicine on an American military base in japan. Good luck in your endeavors!!!!
The primary hurdle is not the language but rather licensing. You will not be licensed to practice medicine in many foreign countries unless you go through their licensing process.
You can however work in the third world where there are no regulations restricting the practice of medicine. In fact-- one of the most interesting guys I know is an MD who teaches 9 months a year and then spends the remainder of his time wandering around places like Afghanistan, sub-Saharan Africa, and Mongolia fixing hairlips and the like. He's one of the few people of whom I am truly jealous.
