The same thing that happened in this Zambia/U.S. inside/out exchange also happened on the next exchange between Zambia/Kenya. Nearly identical.
See the reply beginning about the 4:25 mark. Zambia on the inside and Kenya to the outside coming into the final exchange. Then the hand/baton cross over between the two teams a few seconds after.
The U.S. really was not to blame on that handoff, though it could have been quicker after the block on them. They were completely impeded. The issue was really that they should have been out in front or close to avoid any such nonsense, but were just not running close to their capacity. Considering that McKiver, who has the slowest seed time for the Americans, ran 43.9 in the mixed relay, they should have no problem crushing Kenya. Then they can go with some fresh guys in the final. It might be time to experiment with some 200m guys (and I would say Grant Holloway, a sub-44 split guy in college, if he weren't injured as well) to have a shot at beating Botswana.
